Basic information used to configure your tenant and order headers. This section should be completed by your organization's Super Admin.

AI calibration batch

The last thing we need before we can schedule your Trainer Certification Session.

Please don't upload patient referrals here. The AI Calibration Batch requires 10–20 real historical referrals (orders + clinic notes), which contain protected health information. Do not attach these to this form. Once you submit, our onboarding team will reach out with instructions for sending this batch through a secure, HIPAA-compliant channel.
